M 14 Supervisor Information and Instructions
Prospective Service Project Supervisor
The individual who has asked you to serve as his/her project supervisor is a student of NationsUniversity, a distance learning institution authorized to grant degrees by the Louisiana Board of Regents, U.S.A. The courses offered by the institution are free to all students who reside outside the United States.
As a distance learning school, NationsUniversity services its students through the Internet and e-mail. Some courses are built around local resources and require the student to seek local supervision. Such is the case with the service project. This is why the student has approached you.
BEFORE agreeing to act as a supervisor, you should examine two items: (1) the student’s Project Design to learn the nature and scope of the project and (2) the Supervisor Guidelines so that you will know precisely your duties. Please read these documents carefully and consider your ability to serve in this capacity. Please DO NOT submit your application until the student gives you these two documents, and you have taken the time to read them thoroughly.
As part of this application, you MUST scan and submit a government issued photo ID. If you do not have one, you MAY NOT serve as the supervisor for the student’s project.
By filling out the application, I agree to permit NationsUniversity to verify my credentials and contact me for verification purposes.
Instructions
1. Download the PDF application form to your computer by right-clicking on the link below.
a. If you are using Internet Explorer, after right-clicking on the link, choose “Save Target As”
b. If you are using Mozilla Firefox, after right-clicking, choose “Save Link As”
c. If you are using Google Chrome, after right-clicking choose “Save Link As”
2. Fill in the form. You may type directly into it and save your information.
3. Scan your government-issued photo ID.
4. Return the PDF application form and the scanned ID by e-mail attachment to Dr. Martell Souder at martells@nationsu.org.
